To apply to be a Team Member you will need to submit your application by June 6, 2023:
- Team Application Online (Google Form Online) or Team Application (Print PDF/Mail it)

If you apply to serve as a Team Member, you will be contacted by the Lay Leader or Head Angel.
If you are selected to serve as a Team Member you will need:
- Hesed Medical Release Form (PDF) or
Hesed Medical Release Form (Word) - Team Registration Fee is $100
