Apply to be a Team Member

To apply to be a Team Member you will need to submit your application by August 1, 2025:

• Team Application Hesed Light 2025 Online (Google Form Online – preferred)
  or Team Hesed Light 2025 Application (Print this PDF then Mail it)

If you apply to serve as a Team Member, you will be contacted by the Lay Leader or Head Angel.

If you are selected to serve as a Team Member you will need:

• Hesed Medical Release Form (PDF) or
  Hesed Medical Release Form (Word)
